Students who need medication during the school day must have a medication authorization form signed by both a physician and a parent or guardian. Medication must be delivered to the school nurse in its original labeled container. Families should contact the nurse before sending medication to ensure all required forms and documentation are completed.

If your child has a chronic medical condition, please contact the school nurse as soon as possible. The nurse will work with families and healthcare providers to develop a care plan and ensure that appropriate medications, supplies, and documentation are on file so your child can be supported safely during the school day.

Students must have an up-to-date physical exam and any required medical documentation on file before participating in school field trips or athletic activities. This may include immunization records, medication authorization forms, and health care plans depending on the student’s needs.

Families can send updated physical exams, immunization records, or other health documentation directly to the school nurse by email or deliver copies to the main office. Providing updated records as soon as they are available helps ensure students remain eligible to participate in school activities.
